Patient Discharge Coordination
Supreme Home Care’s Discharge Coordinator will assist in patient’s discharges from Hospitals, Nursing Homes or directly from your Physician’s office.

The client must:
- Have Medicare or private insurance
- Be under the care of a physician
- Have a skilled home health care need that is ordered by a physician and requires the intervention of a skilled Nurse or Physical Therapist, Occupational Therapist, or Speech Therapist.
- Be homebound or require a considerable and taxing effort to leave the home for medical care or an occasional outing.
